The schedule requires weekend on-call coverage from Friday at 5:00 p.m. through Monday at 8:00 a.m., 48 of 52 weekends annually, and all recognized agency holidays.

What you'll do

• Assess patients for program eligibility and manage admission plans of care
• provide patient and family education according to the plan of care
• document and communicate required information in accordance with agency policies and regulatory requirements
• delegate patient-care duties to LPNs and nursing assistants as appropriate
• work independently while managing hospice patient care
• maintain and adjust schedules to support program performance
• promote customer satisfaction and the organization’s mission, vision, and values.
