The Registered Nurse (RN) — Per Diem Home Care role is onsite in Suffolk, VA, providing care in patients’ homes. Compensation is $50 per training, $85 per supervisory visit, and $150 per opening assessment, plus $0.70 per mile. Attend Home Care’s mission is to help people live safely, independently, and with dignity at home. The nurse assesses patients, develops physician-directed care plans, and provides skilled nursing visits and reassessments. The role also educates patients and caregivers, coordinates with physicians and the interdisciplinary team, documents care, and supervises or delegates care to LPNs and home health aides.

What you'll do

• Conduct start-of-care visits and comprehensive assessments
• develop individualized physician-directed care plans
• perform skilled nursing visits and reassessments
• provide wound care, medication management, injections, and chronic disease management for skilled nursing positions
• educate patients and caregivers
• collaborate with physicians and interdisciplinary team members
• maintain clinical documentation
• supervise and delegate care to LPNs and home health aides.
