What you'll do

• Provide skilled nursing care and prescribed treatments in patients’ homes
• focus on pain control and symptom management
• assess patient and family needs and revise care plans
• educate and support patients and families
• participate in interdisciplinary team meetings and care conferences
• communicate with the Clinical Manager and physicians
• document care
• supervise home health aides, LPNs, and other paraprofessionals.
