What you'll do

• Conduct resident assessments and reassessments, monitor changes in condition, and communicate resident needs to the Health and Wellness Director. When necessary, participate in hands-on care, medication administration, and nursing tasks. Schedule and assign caregiving and medication technician shifts
• supervise, train, and support care team members
• assist with physician orders
• maintain resident records
• coordinate care planning with onsite home health agencies
• address resident needs and complaints
• coordinate admissions and move-ins
