What you'll do

• Assess the needs of individuals with developmental disabilities and their families
• develop, implement, monitor, and revise Individual Program Plans
• coordinate associated services and funding
• meet with individuals at home or in the community
• prepare reports and maintain compliant records
• advocate for client rights and access to entitled services
• initiate referrals and monitor vendored services
• identify community service gaps
• and provide intake, outreach, committee, or task-force support as assigned.
