What you'll do

• Serve as a liaison for participants, families, providers, and community partners
• conduct assessments and develop person-centered service plans
• monitor services and address gaps
• provide casework, resource education, and support during critical situations, including hospitalizations
• maintain accurate electronic records and comply with HIPAA, state, and agency standards
• build support networks and conduct stakeholder outreach. Occasional evening and weekend work may be required.
