What you'll do

• Monitor residents’ health and well-being from admission through discharge using the MDS process
• plan, document, and submit compiled information
• communicate with staff, residents, and families
• ensure care plans reflect required care
• maintain healthcare records and reports using PCC
• investigate patient and family complaints and take appropriate action.
