What you'll do

• Conduct daily non-clinical wellbeing checks and assist with intake under supervision
• observe and report changes in client condition
• provide basic health education without clinical interpretation
• support client self-administration of medication under authorized delegation and maintain logs
• coordinate appointments and follow-up
• participate in huddles and team meetings
• document activities in the EHR
• support program metrics, infection control, and safe care areas
• and escalate concerns, errors, refusals, adverse reactions, or discrepancies.
