What you'll do

• Provide one-to-one psychosocial counseling
• complete psychosocial assessments
• coordinate programs and activities addressing patients’ social and emotional needs
• collaborate with the behavioral health team on co-management
• develop and implement interdisciplinary care plans that include patient preferences
• maintain regular contact to review progress and identify additional needs.
