Enrollment Registered Nurse

The Enrollment Registered Nurse works onsite at the High Desert PACE facility and participant homes in Victorville, CA. Pay is $50–$55 per hour. High Desert PACE works with prospective participants and an interdisciplinary team to assess eligibility for its program. The nurse conducts nursing and functional assessments to determine whether prospective participants meet nursing level-of-care requirements. The role coordinates with enrollment staff, clinicians, and state assessors to support eligibility decisions, documentation, and ongoing level-of-care reviews.

What you'll do

• Conduct home visits and center-based clinic appointments
• complete nursing, functional, and home-safety assessments
• determine whether prospective participants meet PACE nursing level-of-care guidelines
• prepare and submit documentation for state review
• track annual state level-of-care reviews
• coordinate with the interdisciplinary and outreach and enrollment teams
• complete eligibility referrals with the primary care physician
• support semi-annual medical-record audits and level-of-care redeterminations
• participate in outreach and business development activities.
