The role helps improve care safety, continuity, and patient health through clinical teamwork and follow-up.

What you'll do

• Support planned primary care through pre-visit planning, huddles, and nurse-led or nurse-assisted visits
• coordinate preventive and chronic disease care with primary care providers
• educate patients and support self-management
• coordinate community care and follow up after emergency visits and hospital discharges
• assist with exams and procedures
• collect specimens, administer ordered medications and immunizations, document care, schedule tests and referrals, and monitor follow-up results.
