Registered Nurse (RN) Case Manager

What you'll do

• Coordinate care, discharge planning, and resource management
• determine appropriate level of care
• review potentially inappropriate admissions, continued stays, and procedures with attending physicians
• identify and report delays
• interview patients and representatives as appropriate
• collaborate with caregivers, social workers, and multidisciplinary teams on discharge plans
• participate in rounds and daily huddles
• coordinate post-discharge services
• evaluate continued hospitalization and document changes
• recommend policy and procedure updates
• report potential problems.
