Family Support Worker

What you'll do

• Develop strength-based service plans
• maintain collaborative relationships with primary care providers and community services
• document client contacts confidentially
• model parent-child interactions
• conduct home visits
• complete assessments and required documentation
• provide information on child development, safety, immunizations, and resources
• facilitate groups
• make referrals
• notify supervisors when a child may be at risk.
