What you'll do

• Assess psychosocial risks, resources, insurance coverage, and care needs
• coordinate care progression and discharge planning with patients, families, physicians, and interdisciplinary teams
• identify and address delays and discharge barriers
• arrange post-acute referrals
• document assessments, interactions, and services
• track avoidable days
• and assist with precepting and mentoring when needed.

Why this role

Requires a Georgia RN license or eNLC privilege, BLS, at least four years of acute-care staff nursing experience, and at least two years of hospital or payer-based case management experience. ACM or CCM is preferred. Associate degree or nursing diploma is listed, with a bachelor's degree preferred.
