Registered Nurse (RN) Case Manager, Home Health and Hospice

The Registered Nurse (RN) Case Manager, Home Health and Hospice role is onsite in Tucson and Oro Valley, AZ. The schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with one 24-hour on-call shift per month; compensation is $40–$45 per hour. The role supports Aegis Healthcare’s Home Health and Hospice Nursing Teams, serving patients in their homes. The case manager assesses patients’ physical, social, and psychological needs, collaborates with patients, families, and the Interdisciplinary Group on individualized care plans, and coordinates care and referrals.
