What you'll do
- Deliver Family Centered Treatment services in home and community settings.
- Guide families through the four phases of the FCT model.
- Coordinate with treatment teams, schools, and medical providers.
- Develop and refine individualized service plans.
- Connect families to community resources and aftercare supports.
- Maintain accurate documentation in the Electronic Health Record.
- Participate in quality assurance activities.
