The Service Coordinator role is onsite in Morrisville, VT, with a Monday–Friday schedule. Compensation is $21.60 per hour. The position supports Green Mountain Support Services’ Developmental Services program for people with intellectual and developmental disabilities. The coordinator helps ensure that services are person-centered, effective, and aligned with individual needs and goals. Through planning, monitoring, home visits, and collaboration, the role supports participants’ health, safety, dignity, and quality of life.

What you'll do

• Develop and implement person-centered plans in accordance with federal and state requirements
• monitor service delivery and effectiveness
• conduct monthly home visits to assess wellbeing and home safety and suitability
• communicate and collaborate with team members in service of individual needs
• protect and promote participants’ health, safety, dignity, and quality of life.
