What you'll do

• Assess clients’ cognitive, physical, emotional, and social needs
• develop and implement individualized care plans
• connect clients with healthcare providers, community resources, rehabilitation services, and applicable HHS Waiver Program services
• conduct follow-ups, home visits, and reassessments
• advocate for clients and help navigate service systems
• explain services and eligibility
• collaborate with interdisciplinary teams
• maintain confidential records in accordance with HIPAA and agency policies.

Why this role
