What you'll do

• Build referral relationships with hospital case managers, discharge planners, physicians, and clinical teams
• identify hospice-appropriate patients and facilitate transitions of care
• educate hospital partners, patients, and families about hospice eligibility and services
• collaborate with hospital teams to improve access to end-of-life care
• and support referral growth through clinical partnerships and responsive service.
