What you'll do

• Assess residents’ health needs and maintain individualized care and service plans
• monitor changes in condition and communicate clinical interventions
• provide nursing care
• coordinate with health and care teams, physicians, residents, and families
• review care plans, physician orders, MARs, and reports
• coach and train care team members
• support medication management and documentation
• participate in quality initiatives, audits, and clinical reviews
• maintain compliance and required licensure.

Why this role
