What you'll do

• Provide skilled nursing care to hospice patients
• complete comprehensive assessments and individualized plans of care
• manage caseloads and coordinate with physicians and interdisciplinary teams
• educate and support patients, families, and caregivers
• integrate into new teams and workflows
• help stabilize branch operations and promote consistent clinical practices
• maintain timely documentation and regulatory and quality standards
• participate in on-call responsibilities as assigned.
