What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine eligibility and service needs
• develop, implement, document, and revise plans of care
• educate patients and families on self-care and nutrition
• provide preventative and rehabilitative nursing procedures
• apply infection-control precautions
• direct home health aides and LPN/LVNs
• document progress and communicate changes in patient needs.

Why this role
