What you'll do

• Accept and treat patients in the designated geographic area
• evaluate home-based physical therapy needs and develop individualized treatment plans
• establish goals with physicians, patients, and families
• implement customized therapy programs
• instruct family members, PTAs, home health aides, and caregivers
• document visits
• communicate with patients, families, and interdisciplinary team members, including on discharge planning
• follow agency policies and professional practice standards
• travel to patients' homes within the assigned territory.
