The Hospice Registered Nurse (RN), Home Care role is a full-time, onsite position based in Fresno, CA, providing care in patients’ homes. Pay is $52–$59 per hour. Valley Regional Hospice provides support to patients and families during hospice care. The nurse assesses patients and families, develops and updates care plans, provides prescribed nursing interventions and symptom support, and coordinates care with physicians and the interdisciplinary team. The schedule is Monday through Friday during daytime hours, with participation in a shared on-call rotation; calls outside business hours are triaged, and an on-call clinician responds when a visit is needed.

What you'll do

• Assess patients and families and document health history and ongoing needs
• provide nursing care using the nursing process
• develop and revise care plans with patients and families
• administer prescribed medications and treatments
• provide counseling and support with daily living and comfort care
• coordinate assigned caseloads when acting as case manager
• maintain clinical notes and communicate changes to physicians
• teach patients and caregivers
• coordinate with community health contacts and the interdisciplinary group
• participate in on-call duties
• supervise ancillary personnel when required
• support quality improvement activities.
