What you'll do

• Remain available by telephone and email during assigned shifts
• conduct urgent physical and psychosocial assessments
• provide nursing care focused on pain and symptom management
• guide patients and families regarding diagnosis, medications, treatments, and progress
• document care
• communicate changes and needs to physicians
• collaborate with the interdisciplinary group, vendors, hospitals, and community residential staff
• conduct after-hours hospice admissions as necessary.
