What you'll do

• Conduct Start of Care admissions using OASIS
• complete follow-up and evaluation visits as needed
• visit patients in the designated territory
• reconcile medications and provide wound care, treatments, and procedures
• monitor patient conditions and report changes
• collaborate with physicians and agency staff
• document observations and interventions
• initiate and administer IV medication infusions in accordance with protocols.
