Clinical Care Leader (LTC)

Utilizes the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care. Collaborates with other inter-professional colleagues to plan, implement, and evaluate care. Documents resident history, symptoms, medications, and care provided. Utilizes technology to enhance resident care and collaborate with team members.
