Registered Nurse

What you'll do

• Assess individuals referred to the program and collaborate on community placement
• develop and implement service plans and nursing care plans
• maintain medical records and medication administration records
• coordinate with physicians and specialists
• promote health and wellness
• administer medications
• oversee high-risk medical conditions and private duty nursing
• train clients, mentors, and staff in medical skills
• participate in Individual Health Plan or Plan of Care teams
• provide consultation on home care and durable medical equipment.
