Home Health and Hospice Registered Nurse is an onsite role based in Heppner, OR, providing care in patients’ homes. Pay is $43.03–$67.14 per hour. The nurse is part of the home health and hospice interdisciplinary team at Pioneer Memorial Home Health & Hospice, part of Morrow County Health District. The role provides coordinated, safe, compassionate, evidence-based skilled nursing care to patients and families according to individual needs, physician orders, and agency protocols. It also contributes to patient education, recordkeeping, and the maintenance of policies, procedures, and clinical skills.

What you'll do

• Provide direct and/or indirect skilled care to patients in the home setting per physician order and agency protocol
• deliver coordinated, safe, compassionate, therapeutic, evidence-based care based on patients’ physical, emotional, and spiritual needs
• maintain relationships with other departments and healthcare professionals
• participate in education, record maintenance, and updates to policies, procedures, and skills
• practice according to Oregon’s Nurse Practice Act and applicable ANA ethics, scope, and standards.
