Access TLC is seeking a compassionate and motivated ECM Care Coordinator to join our San Fernando team. As an ECM Care Coordinator, you will provide primarily in-office case management services while conducting regular field visits to patients in the community. You will play a key role in coordinating care, connecting patients to community-based services, and ensuring timely and efficient delivery
