What you'll do

• Conduct admission, care management, and follow-up visits
• assess patients and develop and update care plans with the interdisciplinary team
• provide physical assessments, medication management and administration, wound care, IV therapy, tube feeding, equipment setup and training, patient education, and comfort care
• coordinate with clinicians and update physicians
• supervise LPNs and aides as needed
• travel to patient homes or facilities
• document care and comply with HIPAA and applicable regulations.
