Chaplain (PRN)

The Chaplain (PRN) role is onsite in Conroe, TX, providing spiritual care in patients’ homes; compensation is not specified. The chaplain assesses spiritual needs and provides counseling and support to terminally ill patients and their families. The role also coordinates with local clergy and other spiritual resources and participates in care planning and interdisciplinary collaboration to support continuity of hospice care.

What you'll do

• Assess patients’ and families’ spiritual needs
• provide spiritual support and counseling
• facilitate visits by local clergy, pastoral counselors, or other support people
• participate in care planning and quality improvement
• collaborate with the interdisciplinary group/team
• maintain required continuing education and knowledge of eligibility requirements.
