Hospice Liaison

The Hospice Liaison is an onsite, field-based role located in Newberg, OR. Compensation is not specified for this position, which supports the Serenity Hospice team in growing their presence and referral network. This role exists to develop and maintain consultative relationships with referral sources to increase hospice census. The Hospice Liaison is responsible for facilitating patient referrals, managing intake information, and implementing educational programs to position the company as a preferred provider.

What you'll do

• Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria.
• Assists in the information gathering, education, and form completion aspects of patient intake, admissions, and transition to home.
• Develops a consultative relationship in assigned accounts by making effective customer contacts and presentations.
• Develops and implements marketing and education programs to increase awareness of the company.
