Intake Coordinator

The Intake Coordinator role is onsite in Coon Rapids, MN, paying $21 per hour. Transformation House’s intake function coordinates new client admissions and maintains relationships with prospective clients and referring entities. The coordinator guides prospects through screening and admission by providing program information, scheduling interviews, collecting documentation, and submitting files for review. The role also maintains wait-list, census, and intake records and helps ensure funder notifications and financial responsibility information are handled as required.

What you'll do

• Provide program and admissions information to prospects and referring agencies
• establish and maintain relationships with referring entities
• schedule and conduct intake interviews
• collect screening documentation
• submit client files to the review team
• schedule admission dates
• notify funders within 24 hours of a new client’s date of illness
• manage the wait list
• enter data in Procentive EMR
• determine client financial responsibility and monitor funder changes
• monitor daily census and intake tracking records
• attend staff in-services and meetings.

Why this role

Requires at least one year of experience in a substance use disorder facility and two to three years of related experience, or equivalent combined education and experience; knowledge of specified Minnesota statutes required.
