What you'll do

• Conduct patient assessments and develop individualized care plans
• review and maintain clinical documentation
• coordinate care with caregivers, physicians, case managers, and families
• monitor patient condition and document changes
• ensure care aligns with care plans and clinical needs
• support Medicare and state compliance through documentation and oversight
• perform supervisory visits and quality assurance checks
• participate in chart audits and compliance reviews
• provide clinical guidance to caregivers and staff.
