What you'll do

• Provide clinical and support services to individuals and families with long-term mental illness and substance-use needs
• work with multidisciplinary teams in homes and community settings
• develop, design, and implement treatment-plan goals with clients
• transport clients to medical and community appointments as needed
• document services and communicate with team members.
