Licensed Director of Social Work

What you'll do

• Coordinate and supervise medically related social services
• supervise Social Work Department programs
• participate in interdisciplinary team meetings
• support residents and families coping with illness, disability, treatment, and facility stays
• address advance directives and medical decision-maker determinations
• provide individual, family, and group services
• support resident rights and dignity
• contribute to care plans
• provide grief counseling as indicated
• oversee discharge planning.
