What you'll do

• Support daily living skills such as hygiene, cooking, shopping, and social interaction
• provide transportation for appointments, errands, and community outings
• monitor safety and follow medical and behavioral plans
• complete incident reports and service summaries
• collaborate with care providers and supervisors
• implement individual service plans and behavioral support strategies
• attend required training and meetings
• submit timecards, mileage forms, and vehicle documentation
• assist with crisis intervention and emergency response as needed.
