What you'll do

• Conduct initial psychosocial assessments and in-person reassessments
• participate in interdisciplinary team meetings and care planning
• facilitate and document care conferences and family meetings
• support discharge planning and transitions between residences or facilities
• coordinate case management and communicate with participants, families, facilities, and care teams
• provide counseling and psychosocial support as indicated
• assist with grievances, appeals, and participant rights
• maintain timely documentation and complete required screenings and forms
• support Medicaid eligibility recertification processes.

Why this role

Requires a master’s degree in social work from a CSWE-accredited school, one year of healthcare-setting experience, experience with psychosocial assessments and care planning, and at least one year working with frail or elderly people. Personal transportation, a current driver’s license, good driving record, and auto insurance are required. Travel may be required, including overnight local or out-of-state travel. Sign-on bonus up to $5,000.
