Shelter Case Manager

What you'll do

• Engage youth in developing Individual Service Plans
• assess needs and document case plans
• connect youth with services and facilitate referrals
• monitor progress toward goals
• coordinate with clinical, outreach, and residential departments
• support employment and education goals
• provide basic individual and group counseling
• maintain statistics and reports
• transport clients when required.
