Registered Nurse – Mobile Residential Support

The Registered Nurse – Mobile Residential Support role is full-time and onsite in Hauppauge, NY, supporting community residence programs across Nassau and Suffolk counties. The salary range is $46.15–$49.74 per hour. CN Guidance and Counseling Services provides integrated clinical treatment, counseling, housing, and support for people living with mental health and substance use conditions. The nurse provides direct care and implements treatment plans for people living in the community. The role coordinates medical services, monitors medications and health needs, supports patient education, and documents care in the electronic health record; periodic on-site coverage and availability as needed around the clock are part of the role.

What you'll do

• Coordinate clinic and laboratory appointments, urine analyses, and injections when needed
• reconcile medications
• conduct vital signs and health assessments
• coordinate medical assessments and additional services
• monitor medication effects and regimens
• support medication education
• schedule provider visits and medication renewals
• document client visits in the electronic health record
• and participate in quality improvement training. The posting requires availability as needed 24 hours a day, seven days a week, including periodic on-site coverage.
