What you'll do

• Provide direct support and mentorship to youth
• coordinate services with foster parents
• conduct home visits as needed
• co-facilitate Family Team Conferences
• assist with applications, referrals, school enrollment, appointments, and community resources
• complete life-skills assessments
• facilitate individual and group life-skills training
• maintain and update curriculum
• conduct community outreach
• document services within 72 hours.
