What you'll do

• Provide nutrition assessments for admissions, quarterly and annual reviews, tube feedings, and significant weight changes.
• Create and update care plans for nursing staff, complete MDS documentation, perform nutrition-focused physical exams, and collaborate with the interdisciplinary team during clinical meetings.
