Social Worker — Long-Term Care

The social worker supports residents and families through admissions, psychosocial assessment, counseling when indicated, care planning, and discharge coordination. The role contributes to resident well-being by communicating needs to the interdisciplinary care team and documenting progress and required information.

What you'll do

• Assist with admissions and inquiries
• conduct resident and family interviews
• complete social service assessments and histories
• help develop and evaluate care plans
• provide counseling and therapy when indicated
• document resident adjustment, progress, and significant behaviors
• conduct confidential interviews in suspected abuse investigations
• coordinate discharge planning
• participate in care conferences
• and communicate residents’ social, psychological, and emotional needs to the care team.
