What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to hospice disciplines as needed or document the rationale for not doing so
• maintain accurate and timely patient records
• record pain and symptom management changes and outcomes
• teach patients, families, and caregivers care techniques
• communicate with the clinical manager and care team
• perform assigned on-call responsibilities and services.
