Job overview

What you'll do

  • Assess eligibility and conduct program intakes
  • develop and update action plans, case notes, and participant files
  • provide life-skills development and targeted interventions for clients at risk of eviction
  • connect participants to medical, mental health, and counseling services
  • advocate for services in action plans
  • conduct outreach
  • make referrals to community services
  • coordinate with community partners
  • conduct scheduled unit visits
  • guide applications and redeterminations for SSI, SSDI, Medicaid, Medicare, SNAP, and other benefits
  • distribute and document grant-funded or donated goods
  • maintain HMIS and physical files
  • track and report outcomes
  • participate in trainings and case staffing
  • transport clients in an agency vehicle as directed.
