Family Centered Treatment Specialist

What you'll do

• Deliver Family Centered Treatment services in home and community settings.
• Guide families through the four phases of the FCT model.
• Coordinate with treatment teams, schools, and medical providers.
• Develop and refine individualized service plans.
• Connect families to community resources and aftercare supports.
• Maintain accurate documentation in the Electronic Health Record.
• Participate in quality assurance activities.
