MDS Coordinator

What you'll do

• Coordinate admission, quarterly, annual, and significant-change MDS assessments
• submit assessments accurately and on time
• participate in interdisciplinary assessment meetings
• collaborate on and update individualized care plans
• maintain regulatory compliance and prepare documentation for surveys and audits
• review records and address coding documentation discrepancies
• educate nursing staff on documentation
• monitor case mix and reimbursement trends
• track quality measures
• liaise across departments and participate in QAPI activities.
