What you'll do

• Complete initial and ongoing assessments, including OASIS
• reevaluate nursing needs and care outcomes
• develop, initiate, and revise plans of care
• provide preventative and rehabilitative nursing procedures
• plan client discharge
• communicate with clients, families, team members, and healthcare professionals
• apply infection-control precautions
• coordinate services and report changes in client condition
• monitor cases for payer requirements
• complete documentation
• supervise, teach, and provide clinical direction to LVNs/LPNs and CNAs
• perform on-call responsibilities and services as assigned.
