Registered Nurse (RN) Case Manager

The RN case manager leads resident care as part of a multidisciplinary team, coordinating intake, assessment, discharge planning, and medical and non-medical case management. The role supports residents’ stabilization and transitions to community care through clinical oversight, advocacy, and coordination with outside providers.

What you'll do

• Triage and admit new residents
• oversee resident care, including symptom and medication management and daily personal care
• develop and maintain resident care protocols
• manage cases involving end-of-life care or medical stabilization and transition to the community
• coordinate with outside providers and advocate for residents
• train and supervise resident care staff and volunteers
• accompany residents to medical appointments as needed
• participate in shared weekend and overnight on-call.
